How people lose marks
"I'm a perfectionist". Panels have heard it several thousand times and it evidences nothing. It also frequently reads as an inability to prioritise.
No cost. Excellence questions are strongest when maintaining the standard was inconvenient — it took longer, it was unpopular, nobody would have known. If it was easy, it isn't evidence.
Standards you were told to meet. Following a procedure is compliance. Going beyond it, or noticing it was inadequate, is what's being assessed.
